Photo Consent

May we use your child’s photograph in preschool’s printed publications for promotional purposes?

Can we use your child’s photo in another child’s learning journey (group photo or in the background) and on display boards

These observations are only visible to the families of children included in that observation and authorised nursery staff. Photographs may also be used on nursery display boards to celebrate children's learning and achievements.

Do you give permission for non-identifiable images of your child (e.g. from behind, hands, or while engaged in activities) to be used on the preschool website?

Do you give permission for your child to be included in video recordings, including annual performances and special events?

We may occasionally record videos of children taking part in activities, celebrations, performances, and special events. These recordings may be shared with parents/carers of children involved or used within the preschool to celebrate children's achievements. Videos will not be shared publicly.

Are you happy for your child to appear in the media

Do you give permission for non-identifiable images of your child to be used on the preschool Facebook page?

To protect children's privacy, we will only use non-identifiable images on our Facebook page. This may include photographs of children from behind, their hands, or children with faces blurred so that they cannot be recognised. These images may be used to showcase activities, learning experiences, and events within the preschool.

Medical Consent

May we sign on your behalf any written form of consent required by medical or hospital authorities should emergency treatment, including a surgical procedure or injection, be deemed necessary if you cannot be contacted?

We would only exercise this authority in an emergency where there is a delay in obtaining your signature and, in the opinion of the medical or hospital authorities, waiting for consent would place your child's health, safety, or wellbeing at risk. Every reasonable effort would be made to contact you and your emergency contacts first.

Local Walks

I agree that the preschool can take my child for outings/trips outside of the school grounds under staff supervision.

Suncream

I agree to apply sun cream to my child before attending preschool in hot weather and give permission for preschool staff to reapply sun cream during the day as required.

During hot weather, please provide a named bottle of sun cream for your child. We ask that sun cream is applied before preschool, and consent is given for staff to reapply it as needed during the day.

In the event that your child has not been provided with their own sun cream and the weather is particularly hot, do you give permission for staff to apply the preschool's sun cream to your child?

Information Sharing

If my child attends another setting or childminder, I give permission for information relating to my child's learning, development and wellbeing to be shared between settings.

Do you give permission for relevant discussions, assessments, records, reports (including photographs) and information about your child to be shared with appropriate professionals and agencies working with your child?

Information may be shared with professionals and agencies involved in your child's care, education, safeguarding and transitions, including other settings, Children's Services, Ofsted and Hampshire Safeguarding Children Partnership, where appropriate.
